💼 About This Role

You'll provide home health occupational therapy to adult and geriatric patients in senior living communities, working as part of a multi-disciplinary team to help clients remain independent at home. You'll perform evaluations, develop care plans, and document using a state-of-the-art touch pad tablet.

🎯 What You'll Do

• Make home visits to clients in designated geographic territories.
• Perform thorough evaluations of functional status and ADL performance.
• Develop individualized intervention plans and revise care plans.
• Educate patients and families on rehabilitative care activities.
• Document interventions using electronic medical records on a tablet.

📋 Requirements

• Current Pennsylvania Occupational Therapist license
• Graduation from an accredited occupational therapy program
• Minimum one year of recent work experience as an OT
• Solid computer skills; prior EMR experience preferred
