💼 About This Role

You'll conduct start of care admissions and perform skilled nursing visits for home health patients across Yolo and Sacramento counties. You'll ensure smooth transitions from hospital to home while collaborating with physicians and an interdisciplinary team. This role offers targeted territories with minimal drive time and flexible scheduling.

🎯 What You'll Do

• Complete initial patient assessments and OASIS documentation
• Initiate and administer IV medications and infusions
• Collaborate with physicians and clinical team on care plans
• Monitor patients' conditions and report changes to team

📋 Requirements

• Current and unrestricted CA RN license
• Valid CA Driver's License
• Current BLS/CPR certification
• Experience with OASIS and start of care

• Experience with high acuity patients
• Previous IV medication infusion experience
