Description

As a home care Physical Therapist with BAYADA at Inspira Home Health, you'll be an integral member of a multi-disciplinary team providing rehabilitative care and skilled nursing to clients in their homes throughout Cumberland County, NJ. You'll conduct diagnostic tests, develop individualized care plans, and educate clients and families to promote health and independent living.

Requirements

• Current NJ Physical Therapist license
• Minimum one year recent PT work experience
• Graduation from APTA or CAAHEP approved program
• Ability to work independently and manage time effectively
• Strong interpersonal, organizational, and problem-solving skills

Responsibilities

• Conduct home visits for diagnostic tests and measurements including mobility, balance, and safety assessments
• Implement wound prevention and repair interventions using evidence-based dressings
• Develop and execute individualized care plans with manual therapy, gait training, and balance interventions
• Assess and revise care plans, coordinating with nursing in multidisciplinary care
• Document observations and evaluations using a touch pad tablet
