💼 About This Role

You'll provide home health visits to adult and geriatric patients, performing evaluations and therapeutic interventions for speech, language, and swallowing difficulties. As a key member of a multidisciplinary care team, you'll help patients remain at home with rehabilitative care. You'll use a state-of-the-art touch pad tablet for documentation.

🎯 What You'll Do

• Make home visits in designated territories.
• Perform evaluations for speech, language, communication, or swallowing.
• Develop and revise treatment plans based on evaluations.
• Educate patients and families on rehabilitative care.

📋 Requirements

• Current SLP license in Pennsylvania.
• Certificate of clinical competence in speech from ASHA or equivalent.
• Solid computer skills.
