💼 About This Role

You'll coordinate housing solutions for high-need students within Amity Foundation's Enhanced Care Management program, acting as a liaison between the care team and County Housing Services. You'll guide students through the housing process, including Coordinated Entry System enrollment and transportation to appointments. This role offers the chance to work with re-entry populations in a supportive therapeutic community.

🎯 What You'll Do

• Collaborate with care management team to integrate housing solutions into care plans
• Liaise between ECM program and County Housing Services for housing searches
• Guide students through housing selection and enrollment into Coordinated Entry System
• Arrange non-emergency transportation for housing-related appointments

📋 Requirements

• Bachelor's degree in behavioral health, human services, or related field preferred
• Two years of experience in housing navigation, social services, or related field
• Valid driver's license, car insurance, and reliable vehicle for local travel

• Bilingual in English and Spanish
• Community Health Worker certification through California HCAI
