Description

As a home care LPN, you will be an integral member of a multi-disciplinary health care team providing rehabilitative care and skilled nursing to clients, allowing them to receive medical care at home. You will perform home visits, administer medication, monitor conditions, and document care using electronic medical records.

Requirements

• Current LPN license in New Jersey
• Minimum one year of recent LPN experience
• Graduation from an accredited nursing program
• Ability to travel to cases
• Strong interpersonal and computer skills

Responsibilities

• Perform home visits to clients in designated territories
• Administer medication, wound care, treatments, and procedures
• Monitor clients' conditions and report changes to clinical managers
• Execute and document doctors' orders
• Accurately document observations and interventions using electronic medical records
