Description

As a home care LPN, you will provide skilled nursing and rehabilitative care to adult clients in their homes throughout Luzerne County, enabling them to receive medical care at home. You'll work as part of a multi-disciplinary team, making visits, performing treatments, and using electronic medical records.

Requirements

• Current LPN license in Pennsylvania
• Minimum one year recent LPN experience
• Graduation from an accredited nursing program
• Ability to travel to cases
• Basic computer skills

Responsibilities

• Making home visits to clients in designated territories
• Administering medication, wound care, treatments, and procedures
• Monitoring clients' conditions and reporting changes
• Performing client assessments and case management
• Documenting observations and care using electronic medical records
