Description

You will provide rehabilitative care and skilled nursing to adult and geriatric clients in their homes, helping them remain independent. As part of a multi-disciplinary team, you will develop care plans, perform diagnostic tests, and educate clients and families.

Requirements

• Current New Hampshire Physical Therapist license
• Minimum of one year recent work experience as a Physical Therapist
• Graduation from an APTA or CAAHEP accredited program
• Ability to work independently and manage time effectively
• Strong interpersonal, organizational, and problem-solving skills

Responsibilities

• Make home visits to clients in designated territories
• Perform diagnostic tests and measurements (mobility, range of motion, strength, balance, etc.)
• Develop and implement individualized care plans including therapeutic exercises, gait training, and balance interventions
• Educate clients and family members on rehabilitative care and activities
• Accurately document client care management using a touch pad tablet
