Description

You will conduct in-home assessments and deliver personalized recommendations for home modifications to help older adults and individuals with disabilities live independently. You'll collaborate with a remote team and document findings through an electronic system.

Requirements

• Degree in Occupational Therapy from an accredited program
• Active, unencumbered OT license
• 2+ years of clinical experience
• Reliable transportation and valid driver's license
• Proficiency with technology and digital documentation

Responsibilities

• Conduct in-home assessments and functional evaluations
• Deliver evidence-based recommendations for home modifications
• Educate clients and caregivers on fall prevention and safe mobility
• Engage clients and families in collaborative care planning
• Maintain accurate documentation through electronic system
