Description

You will provide rehabilitative care and skilled nursing to adult and geriatric clients in their homes, performing diagnostic tests, developing care plans, and educating clients and families. This weekend role involves completing at least four Start of Care visits or 20 productivity points per weekend.

Requirements

• Current Colorado Physical Therapist license
• One year prior clinical experience as a licensed Physical Therapist

Responsibilities

• Make home visits to clients in designated geographic territories
• Perform diagnostic tests and measurements (mobility, range of joints, balance, etc.)
• Implement wound prevention and repair interventions
• Develop and implement individualized care plans including therapeutic exercises
• Educate clients and families on rehabilitative care and independent living
