Description

You will conduct in-home assessments, deliver personalized home modification recommendations, educate clients on fall prevention, and collaborate with a clinical team to support safe independent living for older adults and individuals with disabilities.

Requirements

• Degree in Occupational Therapy from accredited program
• Active, unencumbered OT license
• 2+ years clinical experience
• Reliable transportation and valid driver's license
• Proficiency with technology and digital documentation

Responsibilities
