You'll connect employees to community and company resources, addressing social determinants of health. Your work directly impacts employee well-being by providing one-on-one consultations and referrals. 🎯 What You'll Do
- Conduct one-on-one consultations for work-life, financial, and basic needs support.
- Link employees to appropriate local and national resources via referral lists.
- Document and track metrics including resource needs, referrals, and outcomes.
- Proactively promote community care advocate services to employees.
📋 Requirements
- Bachelor's degree in a related field.
- Proficiency in Spanish.
- Experience working with diverse populations in social work.
- Experience connecting individuals to resources locally and outside local communities.
