You'll provide home health visits to adult and geriatric clients in Bucks County and Northeast Philadelphia. Your core impact is helping clients regain independence through personalized care plans and evidence-based interventions.

🎯 What You'll Do

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements of mobility, strength, and balance.
• Develop and implement individualized care plans with therapeutic exercises.
• Document observations and interventions using a touch pad tablet.

📋 Requirements

• Current PA Physical Therapist license.
• Minimum one year of recent work experience as a Physical Therapist.
• Graduation from an APTA-accredited program.
