💼 About This Role

You'll provide rehabilitative care and skilled nursing to adult and geriatric patients in their homes. Your core impact is helping clients regain speech, language, and swallowing abilities through therapeutic interventions and personalized care plans. This role offers a supportive team environment and flexible per diem scheduling.

🎯 What You'll Do

• Perform home visits to assess and diagnose speech/language difficulties.
• Develop and revise treatment plans based on evaluations and physician orders.
• Educate patients and families on rehabilitative care and activities.
• Document patient care using electronic medical records on a tablet.

📋 Requirements

• Current license as a Speech Language Pathologist in Pennsylvania.
• Certificate of Clinical Competence in Speech from ASHA or equivalent.
• Solid computer skills and experience with electronic medical records.
