You'll provide hands-on therapeutic interventions in patients' homes as part of the largest home health agency in San Francisco. This entry-level field role focuses on follow-up visits with strong leadership support and mentorship.

🎯 What You'll Do

• Provide gait training, balance re-education, and therapeutic exercises in home settings.
• Conduct patient evaluations and implement individualized care plans.
• Document patient care accurately within 24-48 hours.
• Collaborate with Physical Therapist Case Manager and interdisciplinary team.

📋 Requirements

• Current unrestricted California Physical Therapist (PT) License.
• Current BLS/CPR Certification.
• Valid CA Driver's License with clean driving record.
• Graduate of an accredited Physical Therapy program.
