Description

As a home care Physical Therapist, you will be an integral member of a multi-disciplinary team providing rehabilitative care to clients in their homes. You'll perform assessments, develop care plans, and educate clients and families, with additional support during orientation.

Requirements

• Current Pennsylvania Physical Therapist license
• Graduation from an APTA-accredited program
• Ability to work independently and manage time effectively
• Strong interpersonal, organizational, and problem-solving skills
• Solid computer skills; prior experience with EMR preferred

Responsibilities

• Make home visits to clients in designated geographic territories
• Perform diagnostic tests and measurements (mobility, balance, strength, etc.)
• Implement interventions for wound prevention and repair
• Develop and implement individualized care plans including therapeutic exercises and gait training
• Educate clients and families on rehabilitative care and document using a touch pad tablet
