You'll serve as the primary point of contact for clients with complex needs, coordinating care with doctors, specialists, and social services. You'll manage care plans, advocate for clients, and connect them to community resources. This hybrid role offers the chance to make a profound difference every day.

🎯 What You'll Do

• Assess member needs in physical and mental health, SUD, and social supports
• Oversee development of client care plans and goal setting
• Conduct outreach and engagement to facilitate linkage to ECM program
• Complete documentation and maintain patient health records in EMR

📋 Requirements

• Active, unrestricted California LVN license
• 2+ years experience as care manager or community health worker
• Working knowledge of government and community resources for social determinants
• Valid driver's license with clean driving record and reliable transportation

✨ Nice to Have

• Bilingual English/Spanish
• Experience with Google Workspace and MS Office
