Description

You will provide home health visits as a Medical Social Worker, serving adult and geriatric patients in East Baltimore County. You'll be part of a multi-disciplinary team, helping clients remain at home through skilled nursing and rehabilitative care.

Requirements

• Licensed by Maryland Board of Social Work Examiners (LMSW or LISW)
• Master's in Social Work from CSWE-accredited program
• At least 2 years of community-based experience
• Ability to read, write, and communicate effectively in English
• Ability to travel to cases as assigned

Responsibilities

• Make home visits to clients in designated geographic territories
• Provide clinical social evaluations and plan appropriate interventions
• Identify community resource referrals for client and family needs
• Advocate for clients and families to navigate community systems
• Develop and maintain contracts with public and private agencies
