💼 About This Role

You'll provide skilled home health nursing to seniors, collaborating with an interdisciplinary team to empower patient independence. This role skips OASIS assessments and offers hourly pay including travel time, plus flexible scheduling and built-in documentation time.

🎯 What You'll Do

• Conduct home health nursing assessments to determine equipment needs and participant goals.
• Participate in IDT meetings to develop and update participant care plans.
• Deliver and document nursing interventions per care plans, including medication administration.
• Instruct participants and caregivers on disease process, self-care, and prevention strategies.

📋 Requirements

• Graduate of an accredited school of nursing.
• Unencumbered RN license.
• Minimum one year nursing experience with frail or elderly population.
• Reliable transportation.

• Home health agency experience.
• CPR and First Aid certification upon hire preferred.
