💼 About This Role

You'll provide continuity of care as an Advocate Nurse, handling calls and coordinating telehealth for frail elderly participants in the PACE model. This role is fully remote and focuses on building relationships rather than short-term care.

🎯 What You'll Do

• Handle inbound and outbound calls within scope of practice
• Coordinate telehealth meetings between participants and clinicians
• Resolve logistical issues like transportation and medication delivery
• Support care transitions between hospitals and skilled nursing facilities

📋 Requirements

• Graduate of an Accredited School of Nursing with unencumbered RN license
• Nursing knowledge to treat frail, elderly participants and manage complex clinical situations
• Ability to work assigned shifts including days, evenings, nights, weekends, and holidays
