Description

You will provide compassionate home health care to adult and geriatric clients on a per-visit basis, performing OASIS start of care assessments, administering medications, wound care, and other treatments while coordinating with the care team. You'll document using a touch pad tablet and work a part-time or per-diem schedule in the Jacksonville Beach, FL area.

Requirements

• Current RN license in Florida
• Minimum one year recent clinical (medical/surgical) experience
• Graduation from accredited nursing program
• Ability to travel to cases
• Florida Background Screening Clearinghouse compliance

Responsibilities

• Follow care plans and make home visits
• Administer medications, wound care, treatments, and procedures
• Monitor clients' conditions and report changes
• Perform client assessments and case management
• Document observations and care using a touch pad tablet
