🎯 What You'll Do

• Deliver primary care via office and telehealth visits
• Collaborate with physicians and care team members
• Complete annual wellness visits and risk assessments
• Meet quality metrics including STARS/HEDIS

📋 Requirements

• 3+ years of primary care experience as Nurse Practitioner or Physician Assistant
• Experience with HCC documentation and ICD-10 coding
• Active, non-probationary state medical license
• US work authorization
