Description

You will assess residents' health, develop and update care plans, administer medications and treatments, and collaborate with the interdisciplinary team. Your work ensures high-quality care in a supportive environment focused on continuous improvement.

Requirements

• Accredited nursing school graduate with current RN license in state
• CPR/AED certification (or willing to obtain within 6 months)
• Long-term care experience preferred
• Critical thinking and excellent communication skills
• Ability to stand/walk for long periods, lift heavy objects, and transfer patients

Responsibilities

• Assess residents' health status and environmental needs on admission and ongoing
• Develop and revise dynamic care plans through interdisciplinary approach
• Administer medications, treatments, and IV fluids safely and timely
• Communicate changes in condition to physicians using SBARR
• Monitor care delivery effectiveness and resolve problems via QAPI process
