💼 About This Role

You'll manage remote-patient monitoring programs, triaging vital signs and coaching patients toward better health. Your work reduces hospital admissions by providing early intervention through a Remote Care Platform. Compensation: $22-$25/hour.

🎯 What You'll Do

• Manage physiological markers like blood pressure/weight/blood glucose
• Perform monthly wellness calls with assigned patients
• Lead collaborative wellness calls to define health goals
• Manage patient messaging and alerts

📋 Requirements

• Unrestricted RN/LPN/LVN license in a compact state
• Ability to work EST zone between 8:00a - 5:00pm
• 1-3 years of clinical experience or patient management
• Experience with Medicare patients

• Spanish speaking
• Motivational Interviewing/Health Behavior Change experience
• Health Coach certification
