💼 About This Role

You'll provide home health visits to adult and geriatric patients in Hudson County as part of a multi-disciplinary team. You'll perform thorough evaluations, develop individualized intervention plans, and educate patients and families to promote independence. This role offers a $10,000 sign-on bonus for full-time.

🎯 What You'll Do

• Make home visits to clients in designated geographic territories.
• Perform thorough evaluations of functional status and ADL/IADL performance.
• Develop individualized intervention plans and assess home environment for hazards.
• Educate patients and families on rehabilitative care and safety.

📋 Requirements

• Current NJ Occupational Therapist license.
• Minimum one year of recent work experience as an Occupational Therapist.
• Graduation from an accredited occupational therapy program.
