You'll serve as the primary point of contact for clients with complex needs, coordinating care with providers and ensuring access to services. 🎯 What You'll Do

• Assess member needs in physical and mental health, social supports, and referrals.
• Develop client care plans and set SMART goals.
• Coordinate with healthcare providers and advocate for clients.
• Maintain documentation and patient records in EMR/CRM systems.

📋 Requirements

• Active LVN license in California required.
• 2+ years experience as a care manager or similar role.
• Knowledge of government and community resources for social determinants of health.
• Willing to work Monday-Friday 8:30am-5:00pm PT with possible evenings/weekends.

✨ Nice to Have

• Bilingual (English/Spanish).
• Experience with motivational interviewing and trauma-informed care.
• Familiarity with Google Workspace and MS Office.
