You'll provide rehabilitative care and skilled nursing to adult and geriatric clients in their homes, enabling them to remain independent. 🎯 What You'll Do

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements of mobility, strength, and balance.
• Implement wound prevention and repair interventions.
• Develop and adapt individualized care plans with manual therapy and gait training.

📋 Requirements

• Current Arizona Physical Therapist license.
• Minimum of one year of recent work experience as a Physical Therapist.
• Graduation from an APTA or CAAHEP accredited program.

• Prior experience with electronic medical records (EMR).
• Strong computer skills.
