💼 About This Role

You'll serve as the primary point of contact for clients with complex needs, coordinating care across providers to ensure comprehensive support and quality days at home. You'll make a profound difference by empowering clients through evidence-based practices and community linkages.

🎯 What You'll Do

• Assess member needs across physical, mental, and social health domains
• Develop and oversee client care plans and goal setting
• Connect clients to community-based services and supports
• Advocate on behalf of clients with healthcare professionals
• Complete documentation and maintain patient health records in EMR

📋 Requirements

• Active, unrestricted LCSW, LMFT, or LPCC license in California
• 1+ year of experience as a care manager, care navigator, or community health worker
• Working knowledge of government and community resources related to social determinants of health
• General computer skills and working knowledge of Google Workspace and MS Office

• Bilingual (English/Spanish)
• Experience with Motivational Interviewing and Harm Reduction
