Description

Conduct home health visits for adult and geriatric patients, performing evaluations, developing treatment plans, and providing therapy for speech, language, communication, and swallowing difficulties. Collaborate with a multidisciplinary team to help clients remain at home.

Requirements

• Current Speech Language Pathologist license in VA
• Certificate of Clinical Competence from ASHA or equivalent education/work experience
• Solid computer skills; EMR experience preferred

Responsibilities

• Make home visits to clients in designated geographic territories
• Perform evaluations to assess speech, language, communication, or swallowing difficulties
• Develop and revise treatment plans based on evaluations and physician orders
• Educate patients and family on rehabilitative care and activities
• Document observations and interventions using electronic tablet
