🎯 What You'll Do

• Manage a primary caseload of hospice patients
• Provide skilled professional nursing care as prescribed by physician
• Coordinate care with patients, families, and interdisciplinary team
• Ensure compliance with Hospice Medicare Conditions of Participation

📋 Requirements

• Valid Ohio Registered Nurse (RN) license
• Minimum 2-4 years direct patient care experience
• Graduate of approved nursing program
• Experience in hospice or case management preferred
