You'll join a compassionate team providing home health care to adult and geriatric clients in Duval County, FL. Your work directly impacts clients' independence and quality of life through speech, language, and swallowing therapy. This role offers meaningful one-on-one patient interactions.

🎯 What You'll Do

• Perform home visits to evaluate speech, language, and swallowing disorders
• Develop and revise treatment plans based on evaluations and physician orders
• Educate patients and families on rehabilitative care and safety
• Document observations and interventions using a touch pad tablet

📋 Requirements

• Current Speech Language Pathologist license in Florida
• Certificate of Clinical Competence (CCC-SLP) or equivalent
• Solid computer skills with prior experience in electronic medical records
