💼 About This Role

You'll serve as the central point of contact for Kaiser referral intake and clinical escalations on weekends, ensuring seamless care coordination. This role is ideal for a clinician with home health experience who thrives in a fast-paced, autonomous environment.

🎯 What You'll Do

• Manage Kaiser referral intake and acceptance confirmation on weekends.
• Assist field clinicians with urgent patient reassignments and schedule gaps.
• Triage clinical escalation calls from field staff, patients, and Kaiser.
• Coordinate open capacity across field team in real time.

📋 Requirements

• Active California LVN or RN license in good standing.
• Minimum 2 years of clinical experience.
• Home health experience strongly preferred.
• Valid California driver's license with acceptable driving record.

• Prior intake, scheduling, or care coordination experience
• Proficiency with CareStitch, Kinser, or similar home health platforms
