💼 About This Role

You'll provide skilled home health therapy as a Physical Therapist Case Manager in Northern Sonoma County. You'll evaluate patient needs, develop care plans, and restore function to help individuals live safely and independently. This role offers autonomous scheduling and a supportive team environment.

🎯 What You'll Do

• Perform in-home evaluations and develop care plans.
• Help patients with mobility and independence.
• Collaborate with physicians and PTAs.

📋 Requirements

• Current unrestricted CA Physical Therapist License
• Valid BLS/CPR Certification
• Valid CA Driver's License
• Knowledge of local community resources
