💼 About This Role

You'll create and implement care plans as a home health RN case manager in the Santa Cruz area, overseeing a small team. Your work directly reduces hospital readmission rates for patients in their homes. This role offers autonomous scheduling and career growth.

🎯 What You'll Do

• Conduct comprehensive patient assessments including SOCs and discharge planning.
• Develop and execute individualized care plans with interdisciplinary team.
• Supervise 2 nurses executing the care plan.
• Provide wound care, medication education, and CHF management in patients' homes.

📋 Requirements

• Current and unrestricted CA RN license
• Minimum one year of nursing experience
• Current BLS/CPR certification
• Experience working with an interdisciplinary team
