💼 About This Role

You'll provide home health visits to adult and geriatric patients in Lackawanna County as part of a multi-disciplinary team. Your work will help clients remain safely at home while receiving rehabilitative care.

🎯 What You'll Do

• Perform home visits for occupational therapy evaluations
• Develop individualized intervention plans
• Assess home environment for hazards and adaptive equipment needs
• Document care using electronic medical records

📋 Requirements

• Current Pennsylvania OT license
• Minimum one year of recent OT experience
• Graduation from an accredited OT program
