💼 About This Role

As a Home Health Physical Therapist Case Manager, you'll evaluate and treat patients in their homes to restore function and independence. You'll collaborate with physicians and PTAs to develop and execute personalized care plans. This role offers flexible scheduling and the support of a Top Workplace culture.

📋 Requirements

• Current unrestricted CA Physical Therapist License
• Current BLS/CPR Certification
• Valid CA Driver's License
• Knowledge of local community resources

• Home health experience preferred
• Experience with high acuity patients is a plus
• Case management fundamentals highly desired
