Description

You will provide home health speech therapy services to adult and geriatric patients in Washington County, MD. As a key member of a multi-disciplinary care team, you will conduct evaluations, develop treatment plans, and educate patients and families to support independent living.

Requirements

• Current Speech Language Pathologist license in MD
• Certificate of Clinical Competence in Speech from ASHA or equivalent educational/work experience
• Solid computer skills; EMR experience preferred

Responsibilities

• Make home visits to clients in designated geographic territories
• Perform thorough evaluations for speech, language, communication, or swallowing difficulties
• Develop treatment plans based on evaluation and physician's orders
• Revise care plans and participate in multidisciplinary care planning
• Document observations and interventions using a touch pad tablet
