You'll conduct in-home health risk assessments for Porter, a healthcare transparency company expanding into Michigan. Your core impact will be identifying diagnoses and developing care plans for members. This role offers flexible scheduling and paid training.

🎯 What You'll Do

• Visit members to conduct in-home health risk assessments
• Evaluate past medical history and review systems
• Perform medication reconciliation and comprehensive physical exam
• Formulate list of current and past medical conditions

📋 Requirements

• Valid unencumbered Michigan NP license
• 3 years of patient care experience
• Experience in primary care/adult/geriatric
