Description

As a Lead Care Manager at Pair Team, you will directly engage with individuals who have serious mental illness, substance use disorders, or high medical needs, helping them access whole-person care. You'll work mostly remotely but make 2-3 community visits per month. Your work reduces hospital visits and improves outcomes for underserved populations.

Requirements

• Bilingual in Spanish and English
• Experience working with underserved populations
• Strong interpersonal and communication skills
• Ability to work remotely and travel for community visits
• Passion for mission-driven healthcare

Responsibilities

• Outreach and engage individuals with serious mental illness, substance use disorder, or high medical needs
• Coordinate whole-person care including clinical, behavioral, and social support
• Participate in interdisciplinary care team meetings
• Conduct 2-3 monthly in-person visits in the community
• Use AI and automation tools to reduce administrative burden
