Description

You will build relationships with referral sources and hospitals, educate patients about home health services, manage referral intake, and coordinate the transition to home care.

Requirements

• Current license as RN, LPN, MSW, PT, or OT
• Bachelor's degree preferred
• Strong interpersonal skills
• Knowledge of Medicare COPs
• 1+ year healthcare experience, preferably home health

Responsibilities

• Develop relationships with referral sources, physicians, and healthcare professionals
• Market and sell BAYADA services
• Review medical documentation and determine home care eligibility
• Coordinate with service office to set up ordered services
• Complete weekly referral log
