Description

You will support patients across 65 primary care practices in Rhode Island by coordinating referrals, scheduling appointments, and facilitating behavioral health interventions. You'll play a key role in improving patient access and continuity of care, working closely with clinicians and care teams.

Requirements

• 2–3 years in the medical field, preferably Medical Assistant or Quality Assistant background
• Experience with patient-facing roles, particularly telephonic outreach
• Experience in primary care practices
• Experience with value-based metrics around quality and cost containment
• Experience with EHRs, particularly Epic, eClinicalWorks, and Athena
• Comfortable with changing day-to-day priorities
• Excellent verbal communication and interpersonal skills
• Detail-oriented, self-motivated, and comfortable in ambiguity
• Proficiency in Microsoft Office Suite
• Bachelor's degree in healthcare related field preferred

Responsibilities

• Act upon referrals ordered during office visits and facilitate appointment scheduling and referral entry in insurance-based systems
• Ensure accurate documentation of referrals within EMR from initiation to closure
• Schedule appointments internally and with outside specialists
• Facilitate behavioral health referrals and appointment scheduling
• Field calls for patients seeking a new primary care physician
• Facilitate Uber Health rides via the Uber Health Portal
