Requirements

• Active LPN license in Minnesota
• Graduation from an accredited nursing program
• Ability to work flexible shifts (8-12 hours)
• Compassionate and client-focused mindset
• Reliable transportation for home visits

Responsibilities

• Provide 1:1 skilled nursing care to clients in their homes
• Administer medications and treatments as prescribed
• Monitor client condition and document in electronic charting
• Collaborate with clinical team and client families
• Adhere to care plans and state regulations
