💼 About This Role

You'll provide virtual primary care to Medicare patients with chronic conditions as part of a telemedicine team. You'll conduct assessments, manage care plans, and collaborate with RNs and community health workers to improve outcomes for underserved populations. This role is fully remote and supports patients across multiple states.

🎯 What You'll Do

• Provide virtual clinical assessments for Medicare patients
• Manage chronic conditions and medication questions via telemedicine
• Partner with RNs and care managers to close care gaps
• Collaborate with operations to improve care delivery workflows

📋 Requirements

• 2+ active NP licenses in California, Nevada, Pennsylvania, New York, Maryland, Georgia, West Virginia, Texas, Florida, or North Carolina
• Board certification as a Family Nurse Practitioner or Adult-Gerontology Nurse Practitioner
• Bilingual Spanish and English fluency
• 2+ years of clinical experience as a Nurse Practitioner
