Description

You will provide occupational therapy services in senior living communities, performing home visits to evaluate and treat patients's functional status, develop individualized care plans, and educate patients and families to promote independent living.

Requirements

• Current VA Occupational Therapist license
• Minimum one year recent OT work experience
• Graduation from an accredited occupational therapy program
• Solid computer skills; EMR experience preferred
• Ability to work independently

Responsibilities

• Make home visits to clients in designated territories
• Perform thorough evaluations of functional status, including muscle function, endurance, ADLs, and IADLs
• Develop individualized intervention plans to achieve favorable outcomes
• Evaluate home environment for hazards and identify equipment needs
• Educate patients and families on rehabilitative care and document using a tablet
