💼 About This Role

You'll provide virtual urgent and post-discharge care to members with complex medical needs, working in an interdisciplinary care team alongside oncology nurses and social workers. You'll help ensure care alignment with members' goals while supporting clinical quality and program development.

🎯 What You'll Do

• Provide virtual urgent or same-day clinical visits using evidence-based care.
• Conduct virtual post-discharge visits to ensure safe outpatient transition.
• Lead interdisciplinary case conferences and develop care plans.
• Collaborate with external oncology and primary care providers.

📋 Requirements

• Board-certified physician in internal medicine.
• Unrestricted medical license and willingness to obtain additional state licenses.
• Experience in managing medically complex patients and interdisciplinary care.
• Comfort using technology (video visits, EHRs, Google Suite, Slack).
