You will conduct in-home assessments and deliver personalized home modification recommendations for older adults and individuals with disabilities, empowering them to live safely and independently. Collaborate with a remote team and maintain accurate documentation through electronic systems.

Requirements

• Degree in Occupational Therapy from accredited program
• Active, unencumbered OT license
• 2+ years clinical experience
• Reliable transportation and valid driver's license
• Proficiency with technology and digital documentation

Responsibilities

• Conduct in-home assessments and functional evaluations
• Deliver evidence-based recommendations for home modifications
• Educate clients and caregivers on fall prevention and safe mobility
• Engage clients and families in collaborative care planning
• Maintain accurate and timely documentation through electronic system
