💼 About This Role

You'll care for patients over time in skilled nursing facilities, building trust and continuity with vulnerable populations. You'll collaborate with NPs/PAs and use an intuitive EHR with AI Scribe for efficient documentation. This role offers flexible scheduling and autonomy in a mission-driven care community.

🎯 What You'll Do

• Serve as attending physician for patients in assigned skilled nursing facilities.
• Collaborate with NPs/PAs to manage daily patient care and provide clinical supervision.
• Provide patient-centered primary care, diagnosing and treating acute and chronic conditions.
• Participate in Chronic Care Management, Transitional Care, and Remote Patient Monitoring initiatives.

📋 Requirements

• Active MD/DO license in the U.S.
• Board-certified or board-eligible in Internal Medicine, Family Medicine, or related specialty.
• Ability to perform essential job functions with or without accommodation.
