Description

You will provide skilled nursing and rehabilitative care to adult clients in their homes, administering medication, wound care, treatments, and procedures. You'll monitor client conditions, report changes, and document care using electronic medical records. Your work allows clients to receive medical care while remaining at home.

Requirements

• Current LPN license in state of practice
• Minimum one year recent LPN experience
• Graduation from accredited nursing program
• Ability to travel to cases
• Strong interpersonal and time management skills

Responsibilities

• Make home visits to clients in designated territories
• Administer medication, wound care, treatments, and procedures
• Monitor clients' conditions and report changes to management
• Execute and document doctors' orders
• Perform client assessments and case management
