You'll deliver telephonic care management to members with behavioral health and substance use needs as part of a remote clinical team. You'll create care plans, coordinate resources, and guide members through complex healthcare episodes to improve health outcomes.

🎯 What You'll Do

• Deliver virtual care management by phone or video
• Create personalized care plans with members and care team
• Help members navigate medical conditions and benefits
• Coordinate resources to address clinical and social needs

📋 Requirements

• BSN required
• 5+ years of nursing experience
• 2+ years in care, case, or disease management
• Active Compact RN license in good standing

• Active California Nursing License
• Spanish speaking
• Experience with remote work and multiple computer systems
