Description

You will provide in-home physical therapy services to adult and geriatric clients in Cape May County, NJ. As part of a multi-disciplinary team, you'll perform assessments, develop care plans, and educate clients to promote independence and quality of life.

Requirements

• Current NJ Physical Therapist license
• Minimum 1 year recent PT work experience
• Graduation from APTA or CAAHEP-approved program
• Strong time management and interpersonal skills
• Computer proficiency; EMR experience preferred

Responsibilities

• Make home visits and perform diagnostic tests and measurements
• Implement wound prevention and repair interventions
• Develop and implement individualized care plans
• Educate clients and family on rehabilitative care
• Document care using electronic tablet
